Overview
AcuityGuard Health provides read-only reimbursement review software and related services for authorized skilled nursing facility personnel. This policy explains how the public AcuityGuard website handles information. Customer clinical data, if later processed under an authorized service relationship, will be governed by the applicable customer agreement, business associate agreement, security terms, and documented access controls.
Information collected on the public website
If you contact us or request a demonstration, we may receive your name, organization, business email address, telephone number, and the message you submit. We may also receive routine technical information used to operate and secure the website, such as request timestamps, browser type, and network diagnostics.
How information is used
- Respond to inquiries and schedule product discussions.
- Operate, secure, troubleshoot, and improve the website.
- Meet legal, contractual, and security obligations.
- Prevent misuse, fraud, or unauthorized access.
Service providers
We may use service providers for website hosting, form delivery, email, security, and business operations. They may process limited information on our behalf under their applicable terms and safeguards. The current public contact form is delivered through Formspree.
Clinical and PointClickCare data
AcuityGuard does not use the public website contact form to collect clinical records. Any future PointClickCare connection will require authorized access, least-privilege read scopes, customer authorization, and the applicable contractual and security controls. AcuityGuard is designed as reviewer support and does not submit claims, write to PointClickCare, or autonomously code MDS assessments.
Retention and security
We retain public inquiry information only as reasonably necessary for the purposes described above and apply administrative, technical, and physical safeguards appropriate to the information. No internet service can guarantee absolute security.
Your choices
You may ask us to correct or delete public inquiry information, subject to legal and operational retention requirements. You may also choose not to submit the contact form and contact us directly by email.
Updates
We may update this policy as the product, legal requirements, or service providers change. The effective date above identifies the current version.
